We are looking for a community hospice admission nurse who will be the first point of contact for our clients and families as they enter hospice, ensuring the experience is personal and individualized while honoring specific client goals.

About the role

The admission RN will provide comprehensive coordination of admissions, beginning with collaboration with our intake department, reviewing referrals for eligibility, and then scheduling and completing in-person admissions. You will collaborate with the client and caregivers around the hospice plan, organizing and directing the initiation of hospice care, including associated charting and care coordination before handing off to the RN case manager and assigned interdisciplinary team members.

Requirements

  • Minimum of four years of experience as a Community Hospice Nurse with a strong job performance record, or minimum of five years of experience in home health nursing, or a combination of other education and experience from which comparable knowledge and skills are acquired.
  • Certification in Hospice and Palliative Care preferred.
  • Current RN licensure recognized by the State of Vermont.
  • Appropriate experience in the specific clinical area.
