Registered Nurse - Hospice Admissions

About the role

Conduct comprehensive hospice admissions assessments and verify eligibility. Obtain necessary consents and develop initial care plans tailored to patients. Coordinate closely with physicians, pharmacists, and interdisciplinary team members to ensure safe medication management and effective symptom control. Maintain accurate EMR documentation and adhere to safety, quality, and regulatory standards. Provide excellent communication with patients and families to support their understanding and comfort.
