About the role

As a Registered Nurse for Hospice Admissions, you will play a key role in ensuring patients receive compassionate, high-quality end-of-life care from the moment they enter our program.

Responsibilities

• Conduct comprehensive hospice admissions assessments
• Verify patient eligibility and obtain necessary consents
• Develop initial care plans tailored to patient needs
• Coordinate with physicians, pharmacists, and interdisciplinary team members
• Ensure safe medication management and effective symptom control
• Maintain accurate documentation in EMR systems
• Adhere to safety, quality, and regulatory standards
• Collaborate within a growth-oriented team environment
