Registered Nurse (RN), Hospice Admissions

About the role

Admissions RNs at Phoenix Hospice guide patients and families from curative care into comfort-focused care with compassion, clinical excellence, and dignity.

Responsibilities

• Guide patients and families through hospice care transitions
• Collaborate with intake coordinators, hospice consultants, medical directors, facility staff, and physicians
• Provide education on conditions such as CHF, COPD, and Diabetes
• Perform wound care, medication management, change dressings, and medical device management
• Accurately document patient conditions and communicate changes to physicians
• Partner with interdisciplinary hospice teams to deliver seamless care

Requirements

• Hospice experience required
• Active Missouri or Compact RN license
• Strong assessment, organization, and communication skills
• Valid driver's license
