RN / Registered Nurse - Admissions - Hospice

Responsibilities
- Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
- Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
- Obtain medical history, current medications, and symptoms
- Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
- Coordinate the start of services (nursing, aides, equipment, medications)
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
Schedule
PRN, 10-hour shift, every other weekend (Saturday/Sunday).
Requirements
- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year of hospice nursing experience required; 3+ years preferred
- Similar setting experience (e.g., Long-Term Care, Intensive Care, or Emergency Room) will be considered in place of hospice nursing experience
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
