As an RN Admissions Nurse at Ambercare Hospice in Albuquerque, NM, you will be the first clinical point of contact for patients and families entering hospice services. You will conduct comprehensive admissions assessments, facilitate a smooth transition to care, and provide education and support during an important stage of the patient's journey. Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs, and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports, and prepare care plans in a timely manner
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Requirements

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Current CPR and First Aid Certification
• Knowledgeable of quality assessment and assurance procedures
• Valid driver’s license and proof of insurance
