Registered Nurse Admissions
About the Role
As a Hospice Admissions Nurse, you’ll be the face of our care—helping families say “yes” to compassionate end-of-life support when it matters most. You will report directly to the Admissions Manager and serve as a key ambassador for our hospice services.
Responsibilities
- Visit patients and families in their homes, hospitals, or facilities to assess eligibility for hospice care.
- Explain hospice philosophy and services clearly, compassionately, and with clinical confidence.
- Guide patients and their loved ones through the admissions process, ensuring a smooth and supportive start to care.
- Collaborate with referring physicians, case managers, and hospital or facility staff to coordinate transitions of care.
- Use the nursing process to assess patient needs, support care planning, and document clinical findings.
- Build trust and provide emotional reassurance to patients and families as they make this important decision.
- Accurately complete required documentation and ensure compliance with state and federal regulations.
- Work closely with the clinical and interdisciplinary team to ensure timely admissions and quality care handoff.
Requirements
- Active RN license in the state of employment (or eligible to obtain).
- 1+ year of general nursing experience.
- Compassionate, empathetic, and deeply committed to patient-centered care.
- Strong assessment, communication, and interpersonal skills.
- Comfortable visiting patients in hospitals, skilled nursing facilities, or home settings.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
Preferred Qualifications
- 2+ years of hospice, home health, healthcare admissions, or medical sales.
- Bachelor’s Degree and/or Certified Hospice and Palliative Nurse (CHPN) certification.
- Background in case management, care coordination, or healthcare outreach.
- Experience with electronic medical records and compliance documentation.
