The Providence Center ACT Engagement and Diversion Team is seeking a motivated Registered Nurse to assist in assessing and facilitating tasks for adults with co-occurring conditions, medical complexities, and unstable living arrangements. Experience with the chronically homeless and addiction populations is preferred.

Duties & Responsibilities

  • Triage and assess assigned consumers.
  • Assess and evaluate client health care data.
  • Develop an individualized consumer plan of care.
  • Perform integrated care according to medical protocols.
  • Identify and achieve expected outcomes of care.
  • Implement and evaluate the consumer plan of care.
  • Support consumers with daily living issues and conduct home/community visits.
  • Instruct consumers and families on disease processes, treatment plans, and medication effects.
  • Monitor lab results and document accordingly.
  • Coordinate medication changes with external providers and pharmacy.
  • Follow up with clients post-hospital discharge.
  • Complete medication reconciliations and communicate findings to the treatment team.
  • Collaborate with the treatment team to provide comprehensive care.
  • Perform recordkeeping and complete assessments.
  • Maintain cooperative relationships with consumers, staff, and the public.
  • Communicate with various local agencies and departments.
  • Attend trainings, case presentations, and conferences.
  • Contribute to departmental goals and objectives.
  • Serve on relevant committees.
  • Train new hires and student nurses.
  • Occasionally share on-call responsibilities with other staff.

Requirements

  • Associate’s degree in Nursing required; Bachelor’s degree preferred.
  • Bilingual skills preferred.
  • Rhode Island Registered Nurse (RN) license or Compact License required.
  • Experience in Human Services or Outpatient settings preferred.
  • Experience with psychiatric and substance use disorder patients is a plus.
  • Driving may be required; valid driver’s license, registration, current inspection, and insurance are necessary.
