Duties Description
Candidates from outside or within State Service can apply. As Registered Nurse 2 Community, you will be part of the Discharge Planning Team as the nurse expert and duties will include:
- Coordinating meetings with family members to address barriers related to referrals and admissions of individuals to skilled nursing facility
- Developing and overseeing agreement between Hutchings Psychiatric Center and skilled nursing facility to provide convalescent care
- Conducting site visits to skilled nursing facilities
- Coinciding treatment and developing individualized care plans
- Identifying other supports and personal items to assist the individual living at the skilled nursing facility
Minimum Qualifications
- Licence and current registration as a registered professional nurse in New York State, and One year of post-licensure clinical nursing experience providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
- A bachelor’s degree in nursing can substitute for one year of post-licensure clinical nursing experience; and a master’s degree in nursing can substitute for two years of post-licensure clinical nursing experience. However, neither degree substitutes for the required 1 year of specialized community RN experience.
- Candidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

- Additional Comments: Posted hiring rate includes the sum of the statutory hiring rate for PS&T Grade 20 ($75,193), an Increase Hiring Rate ($9,980), and Geographic Pay Differential ($20,000).
- The posted job rate is the sum of the statutory job rate ($94,121) Geographic Pay Differential ($20,000).

- If you fail to maintain your license and/or registration once you are appointed, action will be taken to remove you from the position.

- If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
