Responsibilities

• Float in a designated region covering schedules and filling in for the local team, with potential consistent assignments at some nursing facilities
• Plan, direct, and evaluate resident and employee health care in long-term care settings
• Examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses
• Perform selected laboratory testing and offer referrals to physicians
• Write prescriptions and perform bedside procedures within scope of expertise, protocols, and available equipment
• Work with the medical director in the care and treatment of residents and staff
• Debride wounds using sharp, enzymatic, autolytic, or surgical methods safely at the point of care
• Apply and manage topical wound care and associated wound management products
• Direct and recommend services to improve wound healing outcomes, including labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure devices, and orthotics
• Interpret diagnostic tests and recommend appropriate follow-up
• Recommend prescriptions for medications, Durable Medical Equipment (DME), or supplies to facilitate wound healing or prevention within scope of practice and collaborative agreement
• Perform diabetic foot ulcer risk assessments for members with diabetes, following ADA guidelines
• Educate patients on topical and systemic management of their wounds
• Educate Primary Care Physicians (PCPs), Case Managers (CMs), or referring physicians on current wound management practices
• Provide regular updates to PCPs, CMs, or referring physicians on patient systemic conditions related to wound care
• Act as a resource for decisions, planning, and policies affecting resident health
• Collaborate with staff to identify resident needs and develop plans to meet them

Requirements

• Successful completion of an approved nurse practitioner program
• Current approval by the Board of Nursing to perform medical acts
• Current active, unrestricted APRN license in the state of application
• Master’s degree in Nursing specializing in Nurse Practitioner programs

Physical Requirements

• Able to stand and walk for 4+ hours
• Able to lift 100 pounds to assist fallen residents
• Able to lift 10 pounds for routine performance of essential functions
• Pushing/pulling 70+ pounds when administering patient care
• Bending, squatting, kneeling, and twisting when administering patient care
• Hearing (aids permitted) for performing physical assessments
• Vision (glasses or contacts permitted) for performing physical assessments
• Depth perception for performing medical procedures
• Fine motor skills for grasping and controlling medical equipment and performing precise procedures such as sterile dressing changes

Travel Requirements

• Regional travel a minimum of 4 days a week
• Travel may be scheduled last minute if coverage is needed
• May require overnight stays

Work Environment

• May be exposed to adverse indoor and outdoor environmental conditions
• Potential exposure to bloodborne pathogens, physical hazards (e.g., needle pricks), and chemical hazards

Pay

$120,000 - $135,000 per year. Bonus plan eligible.
