About the role
Carease Health is a nationwide preventative healthcare company focused on reducing hospitalizations. We partner with Skilled Nursing Facilities (SNFs) to deliver high-quality preventative care services, including Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM). We are seeking a motivated and experienced Regional Manager to oversee service delivery, build strong relationships, ensure regulatory compliance, support on-site teams, conduct facility visits, lead strategy meetings, and resolve technical issues.
Responsibilities
- Ensure successful implementation and day-to-day operation of Carease’s preventative care programs within assigned skilled nursing facilities.
- Act as the primary point of contact for facility leadership, including Directors of Nursing (DONs) and Administrators, to ensure alignment and engagement with Carease’s services.
- Oversee adherence to all program protocols (RPM, CCM, CoCM), ensuring compliance with CMS guidelines and company standards.
- Help integrate Carease providers, care companions, and CNAs into the facility workflow, promoting a cohesive, collaborative care environment.
- Travel regularly to facilities to monitor performance, resolve issues, and reinforce best practices.
- Lead monthly strategy meetings to review key metrics, address facility concerns, and align on upcoming goals and initiatives.
- Coordinate and escalate any tech-related concerns promptly to avoid service disruption.
Requirements
- Experience in Skilled Nursing Facilities (SNFs) is strongly preferred.
- Prior multi-site management or regional leadership experience is a plus.
- Strong interpersonal and problem-solving skills — you’re comfortable navigating both clinical and operational conversations.
- Willingness to travel regularly.
- Ability to work independently while staying highly aligned with corporate objectives.
