What You'll Do
Schedule: Monday through Friday – Rotating schedule of 8:00am-4:30pm and 8:30am-5:00pm
As a Referral Specialist, you'll play a vital role in the care team – supporting providers, connecting with patients, and helping ensure a seamless, compassionate experience every day. As a key member of the ambulatory care team, you will:
- Management and navigation of the centralized referral process.
- Responsible for the coordination and monitoring of discharge planning to provide cost effective care and quality outcomes to members in accordance with their benefit coverage guidelines.
- Participate in education and utilization management activities
