Responsibilities
- Contact patients within 72 hours of discharge from either emergency room visit or hospital stay.
- Perform medication reconciliation with patients.
- Schedule appropriate follow-up appointments.
- Follow-up on referrals to ensure that appointment was kept and results have been communicated back to patient and clinic.
- Request appropriate medical records.
- Avoidance of negative outcomes by administering medication accurately, observing patient response.
- Maintain and coordinate communication among patient, staff, and physician as evidenced within medical record, client, and health care team feedback.
- Report patient needs/programs and observations regarding patient’s condition to the physician.
- Work collaboratively with other members of the healthcare team in coordination of the patient’s care as evidenced by other department feedback.
- Participate in staff development programs.
- Take direction and initiate actions (cross/additional training) that will allow the assumption of cross-functional duties to ensure seamless patient care.
- Participate in continuing education activities and obtain required contact hours.

Qualifications
- 1-3 years relevant clinical experience or current valid medical assistant credentials.
- Prior job knowledge and use of electronic medical records.
- Basic Life Support (BLS) required, must obtain within first 90 days of employment.
