About the Role

The Referral Coordinator at Craniometrix will take ownership of non-clinical care support within non-clinical dementia-care navigation and Medicare GUIDE support for patients, families, caregivers, and providers. This includes referral coordination within dementia care navigation support, ensuring every handoff shows what is complete, what is pending, and who owns the next step.

Responsibilities

• Coordinate the Referral Coordinator workflow by helping the team protect sensitive information throughout the process.
• Review the workflow by routing clinical or policy questions to authorized staff.
• Maintain the workflow by documenting status and next action for authorized teammates.
• Track the workflow by helping the team maintain accurate administrative care records.
• Own the workflow by coordinating routine non-clinical follow-up.

Schedule

Full-time, 100% remote within the United States.

Pay

Base salary: $45,000–$68,000 per year.
