The Referral Coordinator Assistant supports the referral and admissions process by receiving, tracking, organizing, and communicating referral information from hospitals, physicians, case managers, payors, and other referral sources.
Responsibilities
- Assists the nurse liaisons and Admissions Team in ensuring timely processing of referrals
- Manages log documentation
- Performs accurate data entry to facilitate patient admissions
