Summary

Under general direction, coordinates inmate reentry screenings, develops reentry plans, links to services, and collaborates with partners. Requires mental health or substance abuse experience and strong communication skills. Performs special projects and programs as directed by the Sheriff or Command Staff as designated.

Essential Job Functions

- Conducts comprehensive reentry screenings with individuals booked into NCSO Adult Detention Center. - Works cooperatively with inmates and practitioners to identify reentry needs and assist with service linkage outside the facility. Service linkage will consist of care coordination, access to medication, access to benefits (AHCCCS), peer support, and transitional housing. - Meet with individuals pre-release to identify needs and help develop a reentry plan. - Interfaces and coordinates with Detention personnel regarding the facilitation of reentry services for inmates. - Attend community meetings coordinating the programs available through the Sheriff’s Office. - Work directly with inmates to ensure Reentry Services are being delivered and makes program improvements. - Promote and adhere to the mission, vision and values of the Sheriff’s Office. - Ensure strict adherence to HIPAA regulations and maintain patient confidentiality in all interactions, documentation and information sharing. Maintain confidentiality of all work-related issues, records and restricted information. - Commitment to confidentiality, equity, and trauma-informed approaches in all client interactions. - Collaborate with third parties, court personnel, and other criminal justice and behavioral health partners. - Perform “special projects” as assigned by the Sheriff or the command staff.

Knowledge and Skills

- Application of all forms of clerical and office practices and procedures. - Proficient in the utilization of research and report writing. - Understanding of law enforcement activities and details as related to behavioral health. - Use effective communication both verbally and in writing. - Capable of public speaking and presenting using various forms of audio video equipment. - Ability to establish and maintain effective working relationships. - Filing and record keeping. - Ability to work independently and collaboratively in a correctional environment.

Desirable Qualifications

- High School Diploma or G.E.D.; AND a minimum of two (2) years in a setting dealing with substance abuse or mental health issues; AND valid AZ Driver License.
