About the role

The Reentry Services Case Manager is part of the multi-disciplinary team at the CJSC responsible for conducting in-reach to Massachusetts Correctional Institutions and County House of Corrections to publicize, enroll, and case manage clients in services available through the Ralph Gants Reentry Services Program (RGRSP). Services include education, career counseling, treatment, housing support, and community-based referrals. The role involves building rapport, demonstrating empathy, setting professional boundaries, and delivering support to ensure positive change for those returning from incarceration.

Responsibilities

  • Conduct in-reach to Massachusetts Correctional Institutions and County Houses of Corrections to publicize RGRSP services, including education, career counseling, treatment, housing support, and community-based referrals.
  • Develop rapport with individuals returning to the community to facilitate a "warm-handoff" by meeting with the same person they encountered while incarcerated.
  • Greet returning individuals at the CJSC as part of the "warm-handoff" process.
  • Develop and maintain a case file for each RGRSP client, including demographic information, educational/vocational assessments, reentry plans, and risk/need assessments.
  • Manage a caseload of RGRSP participants, coordinating with the multi-disciplinary team to engage clients in appropriate services.
  • Work with the CJSC Public Benefits Case Manager to establish essential public benefits and refer clients to community-based resources.
  • Maintain monthly contact with clients for 18 months.
  • Provide information about mentoring training to community-based programs interested in supporting returning individuals.
  • Coordinate mentoring connections for clients who desire them.
  • Cross-train in actuarial assessment tools for risk/need, treatment planning, and contingency management protocols.
  • Facilitate aftercare or alumni groups for CJSC clients.
  • Collect and document urine screens as needed.
  • Adhere to fiscal policies and procedures, including purchase orders, vouchers, and employee expenses.
  • Ensure the Case Manager’s office meets cleanliness and maintenance standards.
  • Attend assigned meetings and actively participate in the Y’s Annual Campaign.
  • Perform other duties as assigned by supervisor(s).

Requirements

Candidates must meet one of the following criteria:

  • Lived experience: Previous incarceration, criminal history, and/or recovery from substance use disorder, along with relevant educational achievement (e.g., certificate, college credit) and intention to continue education toward an Associate’s degree.
  • No lived experience: Associate’s degree and two (2) years of experience working with justice-involved populations.
  • Bachelor’s Degree or intention to pursue one (preferred).
  • Minimum of two (2) years’ experience working with justice-involved individuals.
  • Willingness to work with high-risk populations, including substance abusers, justice-involved individuals, and trauma survivors.
  • Strong oral, written, and presentation skills.
  • Proficiency in MS Office (Word, Excel, Outlook, and Teams).
  • Organization and case management skills (e.g., case notes, filing, tracking client progress).
  • Working knowledge of community resources (housing, healthcare, food, etc.).
  • Some evening availability required.
  • Travel required for meetings and trainings.
  • Valid driver’s license in good standing.
  • Must pass drug testing upon hire.
  • Ability to sit, stand, bend, and walk during the workday.

Preferred Qualifications

  • One year of experience providing case management services to clients involved with a human services agency.
