Schedule
Monday - Friday, 8:00am-4:00pm

$22.90-$26.51/hour

Responsibilities
- Provide clinical case management to engage, educate, and support patients seeking treatment upon release and their families to motivate and maintain them in culturally relevant behavioral health services in the community
- Engage patients beginning 120 days pre-release, with support continuing up to 90 days post-release to the community
- Work independently and as part of a team
- Assist patients with transportation and housing resources and make referrals as needed
- Work on-site unless in the community working with patients
- Coordinate necessary treatment for various patient needs and communicate with all referral sources and treatment entities using proper releases of information
- Work hours required to meet the business needs of the organization and travel to other programs or locations as needed
Requirements
- Bachelor’s degree in human services or related field, or equivalent experience in the behavioral health field (preferred)
- Case management experience (strongly preferred)
