High Point & Affiliated Organizations is a health and human service agency committed to treating and preventing substance use disorders and mental illness. We offer a range of services including inpatient, outpatient, residential, and community-based services, as well as support for survivors of abuse, violence, and homelessness. Our mission is to help individuals and families achieve personal change and improve their quality of life.
Responsibilities
- Provide emotional and social support to Members.
- Support Members in making positive life changes and developing skills for recovery.
- Aid Members in forming recovery-supportive friendships, reconnecting with family, and connecting with recovery-community networks.
- Facilitate connections to primary and specialty medical, dental, and mental health services, as well as social services.
- Assist Members in preparing for meetings with probation officers, social workers, and child protection/child welfare workers.
- Deliver services on a mobile basis to Members in various settings.
- Create and coordinate a Plan of Safe Care for pregnant and/or parenting Members.
- Support Members around perinatal health, housing, health care, income, and mental health concerns.
- Engage in community events promoting prevention and education.
- Coordinate community referrals and assist patients with additional supports and service coordination.
- Support families and individuals struggling with substance use disorders in underserved and disenfranchised populations.
- Facilitate peer groups in Inpatient settings to increase access to services and promote a collaborative approach to treatment.
- Maintain appropriate level of productivity and billable hours.
Qualifications
- High school diploma and lived experience of substance use and recovery.
- Valid driver’s license, clean driving record, and ownership of a car (preferred).
- Successfully completed trainings and/or coursework to prepare for serving as a Recovery Coach.
