REACH Mobile Crisis Coordinator

Easterseals PORT Health is seeking a compassionate and dedicated full-time Mobile Crisis Coordinator to join our Community Regional Crisis Response (CR2) team in Virginia. You must live in Lynchburg, Bedford, or Appomattox areas and be license-eligible.

About the Role

The Mobile Crisis Coordinator functions primarily to provide clinical and coordination services for the REACH Mobile Crisis Team. Primary duties include on-site and telephonic/telehealth crisis intervention, including a shared rotation of a 24/7/365 crisis line, and crisis stabilization for individuals in their home or other community settings.

Responsibilities

  • Provides rapid response to individuals experiencing a behavioral health crisis.
  • Meets individuals in an environment where they are comfortable to facilitate service engagement, stabilization, and resolution of the crisis.
  • Provides appropriate care, support, and supervision to maintain safety for the individual and others, including crisis intervention, community stabilization, care coordination, and treatment planning.
  • Refers and links to all medically necessary behavioral health services and supports.
  • Coordinates with behavioral health providers throughout the delivery of services.
  • Maintains linkages and relationships with community partners.
  • Ensures completion of all required documentation for Mobile Crisis services, including assessment, treatment planning, and Crisis Education and Prevention Plans (CEPP).
  • Participates in required meetings with REACH Team Members and other applicable parties.
  • Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home and Mobile Crisis supports.
  • Monitors individuals for mental health or behavioral symptoms and provides crisis prevention, intervention, and de-escalation supports.
  • Plans, structures, and prompts social and leisure time activities, including those that build skills for behavior management, stress management, communication, adaptive living, and daily living.
  • Collects and enters required data and produces documentation on administrative and service delivery within established timeframes.
  • Complies with all ESPH policies and procedures, HIPAA guidelines, DBHDS rules, Medicaid/Medicare federal guidelines.
  • Attends all staff and program meetings as requested.
  • Maintains regular attendance and punctuality, manages use of PTO time, and follows all policies regarding time off.
  • Demonstrates Easterseals PORT Health valued behaviors of Accountability, Commitment, and Caring.
  • Completes required training within prescribed timeframes.

Requirements

  • Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP) – Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and at least one year of full-time relevant experience.
  • Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.
  • Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related preferred.
  • Must be registered (or eligible to register) as a QMHP-T with the Virginia Board of Counseling or hold a valid Licensed Mental Health Professional-Resident or Supervisee license.
  • Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days unless they are a Licensed Mental Health Professional.
  • Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred.
  • Knowledge of the MH and I/DD systems of care.
  • Ability to utilize computer word processing and data programs including Microsoft Office Suite and web-based data entry.
  • Must demonstrate capacity to effectively communicate verbally and in written form.
  • Must have a valid, unrestricted, state-appropriate driver’s license and vehicle insurance.

Pre-Employment Screenings and Training

  • Satisfactory background check/screening.
  • Satisfactory Tuberculosis (TB) Screening.
  • Current satisfactory completion of therapeutic interventions training – either Therapeutic Options of Virginia (TOVA) or Crisis Prevention Institute (CPI) approved curricula.
  • Current approved certification to provide emergency CPR/First Aid.

Working Environment and Physical Demands

  • Must possess the ability to develop written communication and review work for accuracy.
  • Ability to communicate effectively with internal and external clients and remain in a stationary position for extended periods.
  • Must be physically able to lift a minimum of 50 pounds, bend, and assist individuals with mobility needs.
  • Must be able to hear within normal ranges and talk to articulate crisis prevention, intervention, and de-escalation supports.
  • Must be able to see with or without accommodation to observe individuals.
  • Facility and community travel will be required, which may include exposure to inclement weather.

Schedule

Full-Time: (3) 12-hour shifts 9P-9A and (1) 4-hour shift, Sun-Wed.

Pay

Competitive salary: $65,000 for this full-time, exempt position.
