Main Responsibilities
- Engage with patients to build trust while you assess their physical, psychosocial, and emotional condition as well as their environment during regular nursing visits.
- Adapts communication style to each individual and circumstance to provide an atmosphere that protects patient dignity.
- Recognizes changes in clinical status, anticipates patient and caregiver needs with a sense of urgency, and reports any changes to the patient or their environment to the interdisciplinary team to ensure the highest level of service and care.
- Requests assistance from supervisor or an RN as needed.
- Work collaboratively with your supervising RN Case Manager to initiate and revise plan and goals of care to ensure patient comfort and family/caregiver support and competence throughout their journey through advanced illness and end-of-life.
- Follows nursing standards of practice and meets or exceeds HOC quality goals to ensure the best possible outcomes for the patient and family from admission through bereavement care.
About the role
This position is based out of Pasadena, MD and covers both Anne Arundel and Prince George's Counties. This position is a night shift, full-time - 3 12-hour shifts per week, 8pm - 8:30am with a weekend rotation required.
Requirements
Non-Exempt, paid hourly + differential.
Qualifications
- Bachelor’s Degree in Nursing (BSN)
- Current Maryland State License as a Licensed Practical Nurse (LPN)
- Current BLS Certification
Skills
- Strong communication skills
- Ability to work independently and as part of a team
- Experience working with hospice patients preferred
