Rapid Re-Housing Case Manager - YHDP

About the role

Absorbs responsibility for the operation of the Rapid Re-Housing and Case Management component of the YHDP program, including referrals, admissions, assessments, and Intake. Completes Individual Service Planning with all Rapid Re-Housing young adults. Utilizes Peer Support Specialists and works in close and cooperative communications with Transitional Supervisor and additional Rapid Rehousing Case Managers to ensure compliance with all aspects of YHDP Performance Benchmarks and Program Standards.

Requirements

• Bachelor's Degree in Human Services or related
• Minimum 21 years of age
• Red Cross Training (agency will provide)
• Infraction-clear NYS Drivers License
• Previous experience with transition age youth, homeless youth or adults
• Experience in housing identification and standards
• Experience in case management and service coordination
• Experience in landlord/tenant negotiations

Qualifications
