About the role
MUSC Community Physicians (MCP) is an entity within MUSC that provides healthcare to patients throughout the state of South Carolina. Under clinical practice guidance of the Social Work Supervisor, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
Responsibilities
- Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
- Completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed.
- Identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge.
- Bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
Requirements
- Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
- MSW licensure by the South Carolina Board of Social Work Examiners.
- Certification in case management preferred.
