Responsibilities
- Conduct assessments and develop individualized service plans.
- Monitor client progress and document outcomes.
- Coordinate services with healthcare providers, families, and support staff.
- Ensure compliance with state and federal regulations.
- Advocate for individuals' rights, goals, and community inclusion.
- Participate in interdisciplinary team meetings and case reviews.
Qualifications
- Bachelor's degree in a human services field (e.g., psychology, social work, special education, or related area).
- Experience working with individuals with intellectual or developmental disabilities preferred.
- Strong communication, documentation, and case management skills.
- Must be QIDP certified.
