Job Duties
- Coordinates overall development & implementation of client-based habilitative programming (e.g., IHP, IPP, individual behavior guidelines)
- Acts as chairperson of inter-disciplinary team meetings to coordinate comprehensive delivery of services (e.g., speech, medical, pharmacy, social services, psychology, psychiatry, vocational, educational)
- Serves as chairperson of team meetings to review, analyze, and modify 30/90 day & annual reviews of client habilitation plans
- Trains staff in effective delivery of training techniques/methods and ensures habilitative services & documentation meet requirements (e.g., Medicaid ICF/IID)
- Reviews programs to determine effectiveness/compliance and initiates modifications/corrective action if necessary
- Conducts periodic rounds of cottages/modules/units/buildings or other active treatment locations to ensure proper implementation of programming and staff compliance with regulations
- Acts as chairperson of team meetings to recommend proposed policies and procedures
- Coordinates client home visits, transfers, and discharges to ensure efficiency
- Assists with coordination of youth admissions and releases

Job Requirements
- At least one year of experience working directly with persons with intellectual disability or other developmental disabilities
- One of the following: Doctor of Medicine or Osteopathy, Registered Nurse, Individual with at least a bachelor’s degree in professional human services category (e.g., Psychology, Education, Anthropology)
