About the role
The Pulmonary Disease Navigator provides education, support, and care coordination for patients with respiratory conditions, working collaboratively with a multidisciplinary team to ensure effective treatment and smooth transition from hospital to home.
Responsibilities
- Evaluate patients with diagnoses such as COPD, asthma, and respiratory failure to determine educational and follow-up needs.
- Assist with discharge planning and respiratory orders.
- Develop and implement personalized care plans, including smoking cessation, medication management, oxygen therapy, and pulmonary rehabilitation referrals.
- Provide educational materials tailored to patient needs and perform follow-up calls.
- Maintain statistical data on disease admissions, readmissions, and outcomes to support quality improvement initiatives.
Qualifications
- Associate’s degree in Respiratory Care and Massachusetts Respiratory Care Licensure.
- Registered Respiratory Therapist (RRT) certification and Basic Life Support (BLS).
- Pulmonary Disease Educator Certification or obtain within six months.
- At least three years of experience as a Registered Respiratory Therapist.
Skills
- Knowledge of cardiopulmonary physiology and respiratory science.
- Effective communication with patients and healthcare teams.
- Ability to assess patient status, develop treatment plans, and perform under stressful conditions.
Physical & Other Requirements
- Ability to stand and walk extensively, transport equipment, and operate respiratory devices.
- Manual dexterity and visual acuity for computer and equipment use.
