About the role

The Pulmonary Disease Navigator provides education, support, and care coordination for patients with respiratory conditions, working collaboratively with a multidisciplinary team to ensure effective treatment and smooth transition from hospital to home.

Responsibilities

  • Evaluate patients with diagnoses such as COPD, asthma, and respiratory failure to determine educational and follow-up needs.
  • Assist with discharge planning and respiratory orders.
  • Develop and implement personalized care plans, including smoking cessation, medication management, oxygen therapy, and pulmonary rehabilitation referrals.
  • Provide educational materials tailored to patient needs and perform follow-up calls.
  • Maintain statistical data on disease admissions, readmissions, and outcomes to support quality improvement initiatives.

Qualifications

  • Associate’s degree in Respiratory Care and Massachusetts Respiratory Care Licensure.
  • Registered Respiratory Therapist (RRT) certification and Basic Life Support (BLS).
  • Pulmonary Disease Educator Certification or obtain within six months.
  • At least three years of experience as a Registered Respiratory Therapist.

Skills

  • Knowledge of cardiopulmonary physiology and respiratory science.
  • Effective communication with patients and healthcare teams.
  • Ability to assess patient status, develop treatment plans, and perform under stressful conditions.

Physical & Other Requirements

  • Ability to stand and walk extensively, transport equipment, and operate respiratory devices.
  • Manual dexterity and visual acuity for computer and equipment use.
