Pueblo: Care Coordinator

About the role

PeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Southern Colorado.

Responsibilities

• Manage an assigned caseload of clients
• Coordinate care with clients, families, caregivers, and community partners
• Complete intake assessments and required client visits
• Support caregiver scheduling and coverage needs
• Help address client or staff concerns promptly and professionally

Qualifications

• Healthcare, case management, or caregiving experience preferred
• Strong communication, organization, and problem-solving skills
• Comfortable working independently, in a fast-paced environment, and as part of a team

Work Setting

• This role includes working from home
• Attending occasional in-office meetings
• Visiting client homes
• Traveling throughout the field (up to 50% of the time)

Pay

Pay Range: $20-$25 an hour
