Responsibilities

• Provide client care
• Develop and coordinate care plans
• Conduct assessments
• Deliver health education
• Collaborate with healthcare providers
• Perform office and home visits
• Mentor others
• Ensure compliance with healthcare standards

Qualifications

• Nursing Bachelor’s Degree or equivalent experience
• RN licensure
• BLS certification
• Bilingual English/Spanish proficiency
• Relevant clinical experience
• Knowledge of therapeutic communication
• Knowledge of medication effects
• Basic computer skills
