Responsibilities
- Providing client care through assessments, vital signs, health history, and developing care plans
- Reviewing referrals and evaluating client needs using screening tools
- Collaborating with healthcare providers to coordinate services
- Conducting office and home visits during pregnancy, postpartum, and newborn assessments
- Offering health education and interventions for prenatal, postpartum, and neonatal clients
- Mentoring nursing students, community health workers, and new staff
- Ensuring documentation complies with regulatory standards
Qualifications
- Bachelor’s in Nursing or equivalent experience with relevant certifications
- RN license for the practicing state, with specific licensure requirements for certain locations
- BLS certification within 90 days
- Preferred bilingual (English/Spanish) skills with proficiency in communication
- Knowledge of nursing procedures, therapeutic communication, and medication effects
- Basic computer skills including EMR, Word, and Excel
