PT Substance Abuse Counselor - Somerset, KY (OTP)

About the role

Crossroads Treatment Centers is a family of professionals dedicated to providing the most accessible, highest quality, evidence-based medication assisted treatment (MAT) options to combat the growing opioid epidemic and help people with opioid use disorder start their path to recovery. Our clinics are all outpatient and office-based, with locations in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia.

Responsibilities

• Provides individual counseling to highly complex patients
• Conducts clinical assessments to appropriately assess patient’s treatment needs
• Designs and updates the patient’s treatment plan, including care coordination, referrals, and discharge planning
• Manages assigned caseload and compliance with clinical treatment requirements
• Collaborates with nurses and providers to provide holistic treatment services
• Facilitates urine drug screening for patients

Requirements

• General experience working with underserved populations in a clinical setting
• Lived addiction experience is a plus
• Qualified candidates will hold one or more of the following certifications/licenses: TCADC, CSW, LPCA, MFT, LCADCA, LMFTA, LPATA, LCADC, LPCC, LCSW, LMFT, LPAT, or CADC

Schedule

Expected hours for this role are 29 hours per week:

• 4:45 AM – 10:30 AM, Monday, Tuesday, Wednesday, and Friday
• 4:45 AM – 12:00 PM, Thursdays

All clinical team members are expected to participate in a Saturday rotation, with frequency determined by current staff count.
