Physical Therapy team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality care is maintained at all times. A state-specific Physical Therapy license is required.

Responsibilities
- Evaluate and develop individualized treatment plans for residents
- Deliver skilled physical therapy services in accordance with physician orders
- Collaborate with interdisciplinary team members to optimize care plans
- Monitor, document, and adjust treatments based on resident progress
- Educate residents and families on therapy goals and outcomes
- Provide skilled physical therapy evaluations and treatments in accordance with physician orders
- Develop and update individualized care plans and therapy goals
- Maintain timely and accurate documentation
- Collaborate with interdisciplinary clinical teams regarding resident progress and needs
- Participate in resident care planning meetings
- Assist with education and training initiatives as needed
- Support a safe, respectful, resident-centered care environment
Requirements
- Doctorate or Master’s degree in Physical Therapy from an accredited program
- Current Vermont Physical Therapy license (or eligibility)
- Strong clinical, communication, and documentation skills
- Passion for working with geriatric and post-acute populations
- Graduation from an approved school of physical therapy required
- Two years’ experience in physical therapy preferred
- Ability to direct others necessary
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
