Responsibilities

• Participates in the frequent monitoring and assessment of the mental and physical health status of people served
• Develops a nursing assessment, comprehensive assessment, and an individualized recovery plan, providing knowledge and expertise from the nursing discipline
• Prioritizes medical needs of persons served, tracks medical outcomes over time, facilitates follow-up for medical conditions when necessary, recommends appropriate screenings (e.g., PSA, mammograms, colonoscopy)
• Teaches skills to support each person’s individualized recovery journey and community integration
• Coordinates services with other community mental health and non-mental health providers, as well as other medical professionals
• Mentors and educates individuals and family/significant others on psychiatric medications, side-effects, and physical health care
• Assesses service recipients’ stated concerns, mental illness symptoms, and behavior in response to medication, monitors for medication side-effects, and educates individuals about their illness and prescribed medications
• Administers prescribed medications and injections, draws blood, and prepares laboratory specimens for delivery, if certified to do so
• Supports persons served to find and maintain employment and/or education/training
• Timely and quality documentation that adheres to DMHAS/Medicaid standards and includes family and/or significant others as permitted by the person served
• Supports family and other members of service recipients' social network to help them manage the symptoms and related consequences of the service recipient's illness, reduce family and social stress, and achieve wellness
• Conducts metabolic syndrome screening, including obtaining HgbA1c, fasting glucose, lipid profile, and blood pressure
• Consults with other service providers and resource people regarding the assessment, treatment, and rehabilitation needs of the person served
• Assesses individuals’ daily living skills and teaches needed skills in areas such as budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self-advocacy, and maintaining housing
• Uses Illness Management and Recovery (IMR) and other evidence-based practices, including stage-wise treatment using Motivational Interviewing, Cognitive Behavioral Interventions, and Teaching Strategies
• Provides education to assist the service recipient and family to relate in a positive and supportive manner

Requirements

• Bachelor’s degree in nursing preferred
• Valid NJ Registered Nurse license
• One-year experience working with individuals with serious and persistent mental illness
• Valid driver’s license required
• No more than one moving violation within the past 12 months
• Vehicle required

Pay

$85,600- $96,300 annually

Schedule

Full time 40 hours, 8:00am-6:30pm flexible on days, 24-hour on-call coverage on a rotating schedule
