Responsibilities
- Participates in the frequent monitoring and assessment of the mental and physical health status of people served
- Develops a nursing assessment, comprehensive assessment, and an individualized recovery plan, providing knowledge and expertise from the nursing discipline
- Coordinates services with other community mental health and non-mental health providers, as well as other medical professionals
- Teaches skills to support each person’s individualized recovery journey and community integration
- Assesses service recipients’ stated concerns, mental illness symptoms, and behavior in response to medication and monitors for medication side-effects during the provision of observed self-administration and during ongoing face-to-face contacts
- Administers prescribed medications and injections, draws blood, and prepares laboratory specimens for delivery, if certified to do so
- Supports persons served to find and maintain employment and/or education/training
- Timely and quality documentation which adheres to DMHAS/Medicaid standards and the inclusion of family and/or significant others as permitted by person served
- Supports family and other members of service recipients' social network to help them manage the symptoms and related consequences of the service recipient's illness, reduce the level of family and social stress associated with the illness, and achieve wellness. Educates the service recipient and family to relate in a positive and supportive manner
Requirements
- Education/Certifications: Bachelor’s degree in nursing preferred, Valid NJ Registered Nurse license
- Experience: One-year experience working with individuals with serious and persistent mental illness
- Driver’s License, Vehicle: Valid driver’s license required, No more than one moving violation within the past 12 months, Vehicle required

Pay
$80,000- $90,000 annually
Schedule
Full time 40 hours (Monday-Friday 8:00am-4:30pm)
