About the Role

Telos Health Systems is a clinical service organization reshaping medical and behavioral health care in skilled nursing and assisted living. We are seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to serve as in-house SNF-ists, providing psychiatric and behavioral health care to geriatric residents in Skilled Nursing and Assisted Living settings. This role is ideal for a clinically confident provider who values autonomy, collaboration, and making a meaningful impact in long-term care.

Telos staffs both Psychiatric Mental Health and General Medicine Nurse Practitioners to improve residents' quality of life and overall well-being while reducing return hospital stays. Here, you can build a rewarding career — not just a job.

Why This Opportunity

  • A rewarding career, not just a job, with a focus on development, collaboration, and a purpose-driven environment.
  • Flexible schedule: choose two nonconsecutive weekdays with daytime hours.
  • Reduce unnecessary hospitalizations and improve residents' quality of life.
  • Deliver behavioral health care to geriatric residents in underserved settings.
  • Serve as the facility's clinical leader, collaborating with interdisciplinary teams and medical professionals.
  • Make a meaningful impact with the support of company equipment, hybrid charting, and administrative assistance.

Responsibilities

  • Serve as the facility's leading provider of psychiatric and internal medicine services and day-to-day clinical leader.
  • Oversee day-to-day care for the entire resident population — short- and long-term — with facility nursing staff support.
  • Provide comprehensive health assessments and differential diagnoses, managing acute and chronic illness under protocol.
  • Perform comprehensive physical examinations of patients.
  • Prescribe all necessary medications and treatments.
  • Conduct monthly stability visits for long-term residents.
  • Evaluate residents with changes in condition and participate in Advanced Care Planning.
  • Develop and implement patient management policies and procedures.
  • Collaborate and communicate with interdisciplinary teams, medical directors, attending physicians, and specialists.
  • Complete accurate documentation and CPT coding.
  • Facilitate referrals to other healthcare professionals and medical facilities.

Requirements

  • Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Active license and DEA registration.
  • Master's Degree (required).
  • Excellent written and verbal communication skills, with the ability to build rapport with patients and their loved ones.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Ability to thrive in a fast-paced environment.
  • Preferred: minimum one year of experience in a Skilled Nursing setting.

Pay

Compensation follows a wRVU pay model. Assuming full-time work (75 visits/week or 15 visits/day), a PMHNP could estimate earnings of $140k - $160k/year. A sign-on bonus of up to $7,500 is offered.

Schedule

Flexible, provider-chosen schedule: 2-5 days per week, two nonconsecutive weekdays with daytime hours.

Benefits

  • W-2 employment with full-time benefits available at 30+ hours/week.
  • Medical and prescription coverage (Blue Cross Blue Shield).
  • Dental and vision (UnitedHealthcare).
  • 401(k) through Nationwide.
  • Voluntary Aflac benefits.
  • 15 days Paid-Time Off and six paid holidays.
  • Occurrence-based malpractice coverage ($1M/$3M).
  • $1,000 annual CEU, DEA, and licensure allowance.
  • Mileage reimbursement.
  • Company-provided equipment, including a Microsoft Surface Pro.
