Psychiatric Mental Health Nurse Practitioner, PMHNP
About the Role
We are looking for experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to serve as “SNF-ists,” providing in-house care for the geriatric population in Skilled Nursing and/or Assisted Living settings. This is a W2, pay-per-encounter (fee-for-service) role with a $10,000 sign-on bonus for a limited time.
Coverage Area & Schedule
Coverage Area: Williamstown, KY
Schedule: 2 nonconsecutive weekdays (Monday–Friday), daytime hours — you choose the days!

Responsibilities
- Act as the leading provider of internal medicine services in the Skilled Nursing Facility.
- Collaborate with the facility’s interdisciplinary team and physicians.
- Oversee day-to-day care for short and long-term residents with assistance from nursing staff and guidance from medical directors and attending physicians.
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments for acute and chronic illnesses.
- Prescribe necessary medications and treatments.
- Perform comprehensive physical examinations.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and specialists regarding patient care.
- Conduct monthly stability visits for long-term residents.
- Evaluate patients exhibiting changes in condition.
- Engage in Advanced Care Planning.
Qualifications
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active license and DEA number.
- Minimum one-year experience in a Skilled Nursing setting (preferred).
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast-paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master's Degree required.
