About the role

We are seeking experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) to serve as in-house SNF-ists, providing psychiatric and behavioral health care to geriatric residents in Skilled Nursing and Assisted Living settings. Responsibilities

  • Serve as the facility's leading provider of internal medicine services and day-to-day clinical leader.
  • Oversee day-to-day care for the entire resident population — short- and long-term — with facility nursing staff support.
  • Provide comprehensive health assessments and differential diagnoses, managing acute and chronic illness under protocol.
  • Perform comprehensive physical examinations of patients.
  • Prescribe all necessary medications and treatments.
  • Conduct monthly stability visits for long-term residents.
  • Evaluate residents with changes in condition and participate in Advanced Care Planning.
  • Develop and implement patient management policies and procedures.
  • Collaborate and communicate with interdisciplinary teams, medical directors, attending physicians, and specialists.
  • Complete accurate documentation and CPT coding.
  • Facilitate referrals to other healthcare professionals and medical facilities.

Requirements

  • Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Active license and DEA registration.
  • Master's Degree (required).
  • Excellent written and verbal communication skills, with the ability to build rapport with patients and their loved ones.
  • Strong leadership experience with emphasis on teamwork in a healthcare setting.
  • Ability to thrive in a fast-paced environment.
  • Preferred: minimum one year of experience in a Skilled Nursing setting.

Schedule

2-5 days per week, two nonconsecutive weekdays, daytime hours — you set the schedule that works for you.

Pay

Offering up to $10,000 sign-on bonus. wRVU pay model; assuming full-time, 75 visits/week (15 visits/day), a PMHNP could estimate they will earn $140k - $160k/year.
