Pay

  • Offering up to $7,500 sign-on bonus
  • Assuming Full-Time 75 visits/week (15 visits/day), a PMHNP could estimate they will earn $140k - $160k/year

About the role

We are looking for experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.

Responsibilities

  • Act as a Skilled Nursing Facility’s leading provider of internal medicine services
  • Collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents
  • Work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
  • Prescribe all necessary medications and treatments
  • Perform comprehensive physical examinations of patients
  • Develop and implement patient management policies and procedures
  • Facilitate referrals to other healthcare professionals and medical facilities
  • Communicate with attending physicians and/or specialists regarding patient care
  • Be comfortable and proficient with documentation and CPT coding
  • Conduct monthly stability visits for long-term residents
  • Perform evaluations for patients exhibiting changes in condition
  • Engage in Advanced Care Planning

Requirements

  • Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Must have an active license and DEA number
  • Previous experience in nursing or other medical fields
  • Minimum one-year experience in a Skilled Nursing setting (preferred)
  • Ability to build rapport with patients and their loved ones
  • Ability to thrive in a fast-paced environment
  • Excellent written and verbal communication skills
  • Strong leadership experience with emphasis on teamwork in a healthcare setting

  • Master's Degree (Required)
