Easterseals, a national nonprofit, supports individuals with disabilities through its outpatient program, offering comprehensive mental health, addiction, and wellness services across New Jersey. The program is backed by a team of compassionate professionals who deliver high-quality, individualized care through both in-person and telehealth services.
Key Responsibilities
- Provide comprehensive outpatient psychiatric care, including diagnostic evaluations, treatment planning, case management, medication management, and health/nutritional counseling for a diverse patient population.
- Preserve and interpret lab results and ensure appropriate physiologic monitoring.
- Manage an assigned patient panel, delivering care via in-person and telehealth modalities while supporting patient access, continuity of care, and program growth.
- Collaborate closely with psychiatrist and multidisciplinary team (LCSW, LPN, care coordinators, group homes, guardians, external providers) to coordinate care, support clinical protocols, and participate in case review and QI initiatives.
- Maintain timely, accurate, and audit-ready documentation in the Electronic Health Record in compliance with HIPAA, Medicaid/Medicare, and all federal, state, and regulatory requirements (e.g., DMHAS, DHS).
- Adhere to organizational safety, infection control, and emergency response protocols and standards.
- Support implementation of clinical protocols governing visit cadence, monitoring, documentation, and quality.
- Support the program's expansion of active patient panel and access targets through efficient, high-quality clinical throughput.
- Uphold professionalism, cultural sensitivity, and confidentiality when interacting with clients, families, and staff, and respond promptly to communications to support effective service delivery.
Requirements
- Active, unrestricted New Jersey Advanced Practice Nurse (APN) license (or eligibility to obtain prior to start).
- Board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) through the ANCC.
- One-year post-licensure experience providing psychiatric assessments and treatment planning to individuals with intellectual and developmental disabilities (IDD).
- Experience in a multidisciplinary, value-based, or community behavioral health setting.
- Experience with and ability to deliver telehealth psychiatric care.
- Demonstrated competence in outpatient psychiatric evaluation and medication management.
- Active DEA registration and New Jersey CDS registration (or eligibility to obtain prior to start).
