About The Organization

SaVida Health is a specialty medical practice providing outpatient opiate and alcohol addiction treatment services. SaVida's care model includes medical care, counseling, comprehensive toxicology screening, case management, and medical management of psychiatric medications. SaVida offers compassionate and individualized treatment programs to support those with substance use disorder.

ESSENTIAL DUTIES

• Identifies patient mental health problems and prescribes treatment
• Implements mental health services utilizing therapeutic regimens
• Works with Clinical Director, Practice Manager, and Medical Director to develop an integrated model of behavioral healthcare
• Provides mental health education to patients, healthcare and medical staff
• Conducts health/behavioral health assessments (including differential diagnoses, prognosis), treatment recommendations, and treatment planning for patients across assigned locations
• Affirms quality and safety of clinical care
• Participates in case reviews and contributes to treatment planning and goal setting
• Communicates with health care providers outside of the practice and interprets health results for patient care planning

Position Requirements

• PMHNP Licensed in ME
• MR software experience (Athena software a plus!)
• U.S. DEA Certificate
• Prior experience with Mental Health and Chemical Dependency treatment preferred
