What You Will Do
- Partners with patients and families to implement plan of care, treatments, and procedures that contribute to the physiological and psychosocial outcomes of the patient.
- Incorporates evidence-based standards of practice.
- Collaborates with interdisciplinary team in the coordination of care.
- Facilitates safe patient transitions to the next care setting as part of the plan of care.
- Demonstrates a sense of urgency to facilitate patient throughput to assure patients receive consistent, competent, and ethical care.
- Focuses on quality outcomes of patient care when addressing customer’s needs to include current evidence-based practice and quality measures.
- Demonstrates effective nursing skills (vitals, blood sugars, administering treatments and medications) in completing tasks accurately and efficiently.
- Follows protocols and physician orders.
- Demonstrates understanding of diverse patient care populations and ability to modify educational approach to ensure patient/family comprehension.
- Evaluates for patient comprehension and documents response.
- Demonstrates the ability to build trusting collaborative relationships with peers and multidisciplinary team to support patient care needs.
- Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements.
- Demonstrates task prioritization appropriately to facilitate quality patient care and ensure patient care needs are met.
- Able to recognize and respond to emergency situations.
- Evaluates own needs for support and uses identified systems/resources to obtain assistance.
Qualifications
- Program Graduate, Practical Nursing (Required)
- High School Diploma or GED (Required)
- 1 year Acute care experience (Preferred)
- Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
