PRS Intervention Case Manager - Central Region - Bilingual

About the role

The PRS Intervention Case Manager is responsible for providing intervention services to clients in the Central Region. The role involves assessing client needs, developing and implementing service plans, and coordinating with other community agencies.

Responsibilities

• Assess client needs and develop service plans
• Implement and monitor service plans
• Coordinate with other community agencies and stakeholders
• Provide counseling and support services
• Document progress and maintain client records

Requirements

• Bachelor's degree in Social Work, Psychology, or related field
• At least 2 years of relevant experience in social services or counseling
• Licenses/certifications in Social Work or Counseling preferred
• Bilingual in English and Spanish

• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite
