PRP Direct Service Worker

About the role

We are currently in search of an experienced Psychiatric Rehabilitation Program (PRP) Direct Service Worker. Direct Service Workers provide assessment and treatment services that promote recovery, full community integration, and improved quality of life for persons diagnosed with serious and chronic mental health conditions. Services are collaborative, person-directed, and individualized, focusing on helping individuals develop skills and access resources to reduce or eliminate maladaptive behaviors stemming from a mental health diagnosis.

Responsibilities

• Work scheduled shift of Monday - Friday, 5:30am - 1:30pm
• Serve as a liaison between the PRP and Mental Health Departments at all CCG sites to ensure consistent collaboration and a constant flow of referrals
• Coordinate and process referrals from MH staff, ensuring completion of the PRP Screening, Letter of Acceptance, and Initial Authorization Request Form
• Coordinate the referral process with Revenue Cycle Staff and maintain consistent communication between departments
• Perform monthly chart reviews, document the process, and provide feedback to staff and leadership
• Audit record keeping in SAMMS to ensure holds are managed and services/forms are completed and signed
• Conduct initial assessments to understand the client’s level of functional impairments, strengths, abilities, needs, and preferences
• Develop an individualized, patient-centered treatment plan outlining goals, objectives, and interventions
• Deliver services both onsite (clinic setting) and offsite (home/community), including:
  • Teaching and instruction
  • Encouraging positive change
  • Skill demonstration and practice
  • Promoting individual wellness, self-management, and recovery
  • Assisting in the restoration and/or improvement of self-care and social skills
  • Facilitating strategies for independent living and self-sufficiency
  • Exposing individuals to academic, vocational, and employment training/resources
  • Facilitating culturally relevant activities for improved social relationships
• Provide services through face-to-face interactions and telehealth
• Complete documentation for treatment planning, monthly summaries, progress notes, and other assigned documents
• Facilitate monthly collaborations with the client’s assigned mental health therapist and treatment team
• Maintain clinical records and compile documentation required by CCG and governing agencies, including signing all forms and services
• Participate in staff meetings, program clinical team meetings, clinical supervision sessions, and in-service as required
• Support the CCG philosophy and treatment modalities
• Ensure confidentiality regarding client issues, background, and treatment
• Exhibit clear, professional, respectful, and effective communication with families and external contacts
• Work independently and implement time management strategies
• Exhibit a high degree of ethical conduct, integrity, and confidentiality
• Ensure client information complies with HIPAA standards
• Maintain case files within the electronic health record (EHR)
• Perform other duties as assigned

Requirements

• At least a high school diploma or equivalent; Associate’s Degree or higher preferred
• Previous experience working with emotionally and behaviorally challenged adults or in a recovery-oriented program
• Experience and proficiency in using an electronic health record system
• Strong interpersonal skills
• Ability to travel in the local area and perform field work
• Validated driver’s license and a dependable vehicle to travel between sites and provide offsite services
• Actively working toward certification provided by the Commission on Rehabilitation Counseling

Pay

$19 - $22 per hour. Factors such as geographical location, relevant experience, licensure, education, and skill level may impact pay.
