Provider Relations and Community Outreach Specialist

Key Responsibilities

  • Build and maintain relationships with pediatricians, diagnosticians, psychologists, schools, daycares, speech and occupational therapy providers, hospitals, social workers, case managers, and community organizations.
  • Identify high-potential referral sources and develop territory-specific outreach plans.
  • Conduct scheduled office visits, introductory meetings, presentations, lunch-and-learns, and follow-up meetings.
  • Clearly communicate Blossom’s services, accepted insurance plans, service areas, referral process, and patient availability.
  • Develop new referral partnerships while strengthening relationships with existing partners.
  • Serve as a reliable point of contact between Blossom and professional referral sources.

Community Outreach

  • Represent Blossom at local conferences, networking events, school programs, resource fairs, and community events.
  • Identify partnership opportunities that improve access to ABA therapy and other resources for families.
  • Cook up and coordinate targeted outreach initiatives with the Director of Marketing and state leadership.
  • Deliver approved marketing and referral materials to priority organizations.
  • Help strengthen Blossom’s visibility and professional reputation throughout the assigned market.

Referral Growth and Follow-Up

  • Generate qualified patient referrals from professional and community relationships.
  • Maintain a structured follow-up schedule with referral partners.
  • Track which organizations are referring, which relationships require additional development, and where new opportunities exist.
  • Communicate referral updates appropriately and help create a responsive experience for professional partners.
  • Work with intake and operations teams to identify and address referral-process barriers.
  • Support referred families without making clinical promises or guarantees regarding insurance authorization, staffing, or start dates.

CRM and Performance Reporting

  • Record contacts, visits, calls, emails, meetings, events, referrals, and follow-up activities in CRM.
  • Maintain accurate referral-source and professional-contact information.
  • Use platform to plan routes and document field visits.
  • Provide weekly updates on outreach activity, developing partnerships, referrals, pipeline movement, and market opportunities.
  • Use CRM data to determine which relationships and territories are producing meaningful results.
  • Track qualified referrals based on families reaching Pending Initial Authorization or a later client-pipeline stage.
  • Track Active Clients separately as the final conversion outcome.

Performance Expectations

  • New qualified professional contacts established.
  • In-person provider visits and meetings completed.
  • Follow-up activities completed on time.
  • New referral partnerships developed.
  • Professional referrals generated.
  • Referrals reaching Pending Initial Authorization or later.
  • Referrals progressing to assessment and active services.
  • Accuracy and consistency of CRM documentation.
  • Growth within assigned territories and priority referral accounts.

Required Qualifications

  • At least two years of experience in healthcare business development, provider relations, referral development, community outreach, outside sales, account management, or a related position.
  • Demonstrated experience building relationships with healthcare providers or professional referral sources.
  • Prominent ability to generate referrals, partnerships, accounts, or measurable territory growth.
  • Strong verbal, written, presentation, and relationship-building skills.
  • Ability to work independently and manage a field-based schedule.
  • Strong organizational and follow-up skills.
  • Experience using a CRM to document outreach and measure performance.
  • Valid driver’s license, reliable transportation, and ability to travel locally throughout the assigned territory.
  • Ability to attend occasional evening or weekend community events.

Preferred Qualifications

  • Experience working for an ABA therapy, autism services, behavioral health, pediatric therapy, home health, hospice, rehabilitation, or other referral-driven healthcare organization.
  • Existing relationships with pediatricians, diagnosticians, schools, therapists, case managers, or autism-focused organizations.
  • Knowledge of ABA therapy, autism services, healthcare referrals, insurance authorization, or patient intake processes.
  • Experience creating and managing a healthcare territory-development plan.
  • Bachelor’s degree in marketing, communications, business, healthcare administration, public relations, or a related field.
