About the role

We are looking for a compassionate, resourceful, and highly skilled Service Navigator to help individuals access services, provide options counseling, and offer hands-on support to connect clients with long-term services and supports.

Supervisor: Lead Service Navigator

Responsibilities

  • Provide timely information, referrals, and application assistance in-person, by phone, email, or home visits.
  • Conduct detailed assessments and offer options counseling to help clients understand and access available services.
  • Collaborate with municipal agents, senior center directors, and community organizations to support client needs.
  • Work with the Benefits Access Team and connect consumers to other WCAAA programs.
  • Complete required certifications (CHOICES, AIRS, and Person-Centered Counseling) within the first 6 months.
  • Participate in outreach presentations, workshops, and trainings as needed.
  • Maintain accurate records, meet annual program targets, and complete required reports.

Requirements

  • Bilingual in Spanish strongly preferred.
  • Knowledge of aging services, long-term options, and caregiver issues.
  • BA/BS degree in human services or related field preferred; Associate’s degree with 5–7 years of relevant experience considered.
  • Direct experience in human services (especially working with seniors) preferred.
  • Strong interviewing, counseling, and communication skills.
  • Proficiency with Microsoft Office applications.
  • Valid CT driver’s license and reliable transportation.
  • Ability to lift/carry 5–10 lbs. and work in diverse environments.
