Responsible For

• Participating in team treatment meetings to represent the therapeutic needs and goals of assigned patients.
• Performing psychosocial assessments on assigned geriatric, adult, and adolescent patients.
• Completing indicated treatment plans using clinical information provided through the multidisciplinary assessment process.
• Meting with patients to discuss treatment plan goals, define discharge criteria and plans, complete family sessions, and assess level of patient risk.
• Providing daily group therapy to geriatric, adult, and adolescent populations as scheduled.
• Serving as a liaison between team members and involved social service agencies and ensuring a clinically appropriate aftercare plan is in place.

Experience Matters

• Education & Licensure: Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
• Licensed to practice in the State of Illinois as a mental health professional is required.
• A minimum of two (2) years’ experience in a psychiatric health care facility, with direct experience in family and group therapies, crisis intervention, and treatment skills; and have strong clinical assessment skills preferred.
