Program Supervisor

About the role

The Program Supervisor provides direct oversight and supervision of the Care Coordination Team, ensuring the program maintains regular Medicaid compliance, manages monthly billing processes, and uses health information technology to inform practice. Together with the Director for Care Coordination and Health Homes, the Supervisor guides the program toward achieving Health Home goals. This position involves significant supervisory, administrative, and program management responsibilities, focusing on client-centered, outcome-oriented care and continuous quality improvement.

Responsibilities

• Provides clinical and administrative supervision to Care Coordinators, including monitoring performance, training, and coaching to improve skills.
• Handles staff recruitment and hiring.
• Assesses staff skill levels and develops individualized training plans, recommending internal/external workshops and trainings for staff development.
• Tracks staff work activities, including outreach, intake, assessment, service planning, referrals, follow-up, collaboration with community-based organizations, documentation, and adherence to contact standards.
• Develops and implements performance improvement plans and manages progressive disciplinary processes as needed.
• Writes and delivers performance appraisals.
• Conducts quality assurance reviews to assess service quality and effectiveness, ensuring compliance with Health Home agreements and proper documentation.
• Develops and implements action plans to improve staff effectiveness and efficiency.
• Oversees care provided by the care management team through case conferences, field visits, and customer service calls.
• Responds to inpatient and ER admission alerts, contacting medical providers/hospitals for admission/discharge information.
• Assists in planning and implementing operational procedures, providing continuous feedback to program management.
• Serves as the point of contact for crisis intervention services and maintains 24/7 availability for staff and client emergencies.
• Ensures staff maintain clinical documentation and records in compliance with HIPAA regulations.
• Utilizes multiple Electronic Health Record systems.
• Maintains client assignments and rosters.
• Reviews all billable contact notes and ensures monthly billing is submitted to the Office Manager by the first business day of the month.
• Responsible for weekly, monthly, and quarterly reporting of program information to the Program Director and Assistant Director.
• Develops, delivers, and participates in ongoing professional trainings.
• Represents the organization in local and state meetings as required.
• Performs other duties as assigned by the Program Director and/or Executive Management.

Requirements

• Licensed Mental Health Professional (LMHC, LMSW, LCSW).
• Excellent communication skills, ability to multi-task, prioritize work, and supervise staff in a culture of accountability.
• 4 years of experience working with the seriously mentally ill to supervise the provision of services to Medicaid recipients with chronic medical and mental health issues.
• Knowledgeable about Health Home regulations and comfortable working with clients in the field.
