Description

Under general supervision, conducts mental, physical and social assessments to provide case planning and determination of eligibility to authorize services to disabled or frail adults.

HS In Home Support Services Social Worker III is the advanced journey-level classification where the incumbent works independently to provide complex services under the general supervision of the lead worker and supervisor. The incumbent demonstrates comprehensive knowledge of In Home Support requirements and the overall program and may serve as a resource for the rest of the team.

EDUCATIONAL INCENTIVE: Possible educational incentive of 2.5%, 3.5% or 5% of base wage, based on completion of an Associate's/Bachelor's/Master's degree.

BILINGUAL INCENTIVE: Incumbents may be eligible for an additional $.65, $.80, or $.90 per hour, based on their qualifications and the operational needs of the Agency.*

Examples Of Duties

  • Assess social, physical, mental functioning and determine the needs of clients through interviews, home visits, record reviews or consultation with appropriate agencies and individuals.
  • Refer, determine eligibility for services, authorize and/or coordinate services as necessary to complete individual service plans. Follow through with case management services to assure that services are provided in a timely and complete manner.

Typical Qualifications

  • Knowledge, Skills, and Abilities:
    • Thorough knowledge of: principles and techniques of interviewing, case management, and crisis intervention; community resources and social services programs available for the disabled; physical and mental illness and their impact on personality and function abilities.
    • Ability to obtain and recognize relevant and significant facts; establish rapport with clients and their families of various ethnic and cultural backgrounds; write effectively utilizing word processing software; analyze situations and adopt effective courses of action; work effectively with representatives of other agencies.
  • Education/Experience:
    • A Bachelor's Degree from an accredited university in a field related to human services AND at least two (2) years of experience comparable to an HS IHSS Social Worker;
    • OR Six (6) years of Human Service experience including two years comparable to an HS IHSS Social Worker.
    • A Human Services Certificate can be substituted for six (6) months of the required experience.

Candidates successfully completing the examination process may be placed on an eligible list for a period of one (1) year.

* Spanish Fluency Examination: For positions requiring bilingual skills, candidates will be expected to take and pass a Spanish/English fluency examination with a score of 70% or higher.
