Provide intensive and specialized supports for people who have an intellectual disability or mental illness, focusing on domains such as personal care, recreation/leisure, vocational, safety, social, and/or community awareness and integration. Offer individual supports to persons with extraordinary behavioral or medical support needs.
Responsibilities
- Provide behavioral coaching to individuals with developmental disabilities or mental illness in assigned treatment areas.
- Complete progress reports and other necessary forms or documents.
- Participate and interact with individuals in various activities as specified in Individual Service Plans and Social Transition Plans.
- Consult with Behavioral Analyst and Qualified Intellectual Disability Professional concerning appropriate goals, objectives, and treatment techniques.
- Assist in the development and execution of goals and objectives for people served.
- Make suggestions for improving organization and program effectiveness and efficiency.
- Complete a monthly report reflecting Program Specialist intervention.
- Complete an annual report, assessed quarterly to track progress.
- Assist with development and implementation of age-appropriate and functional training materials.
- Assist with development of a monthly calendar.
- Work with supervisor in the development of an individual plan of personal growth and development.
- Transport persons served to and from activities as needed.
- Work with independent living coaches in encouraging their professional growth and knowledge.
Requirements
- Bachelor’s Degree in Social Work, Psychology, or related field.
- Valid Driver’s License.
- Pace certification.

- Able to implement goals, objectives, policies, and procedures necessary for providing quality care.

Physical Demands
- Able to lift over 40 pounds from floor to waist unassisted.
- Push and pull up to 75 pounds; push wheelchairs up to 200 pounds.
- Perform physical movements including squatting, stooping, crouching, and kneeling.
- Stand, sit, or remain stationary for extended periods.
- Operate motor vehicles.
- Perform repetitive hand and finger movements for computer and phone use.
