About the role

Mentor Community Services, a part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. The Program Clinical Coordinator plays a crucial role in coordinating services, ensuring quality care, and supporting clients and their families.

Responsibilities

  • Affirms clients and develops a service plan in collaboration with a treatment team and funding sources, adhering to corporate, contractual, and regulatory standards.
  • Evaluates and implements the service plan, ensuring all necessary medical and dental appointments are scheduled and completed.
  • Coordinates the development, review, and revision of programs and behavior intervention plans, and oversees comprehensive support services and oversight.
  • Schedules and provides opportunities for quality training, recreation, and leisure activities, considering clients' choices and preferences.
  • Maintains accurate and complete client reporting records in accordance with corporate and regulatory standards.
  • Assists with legal, licensing, regulatory, and certification activities as needed, including assisting with Mentor training and development.
  • Manages and responds to calls after normal business hours for assigned clients, depending on individual programs.

Requirements

The Program Clinical Coordinator must be able to perform the essential functions of the job either unaided or with reasonable accommodation. They should have a bachelor’s degree in human services or a related field, with one year of related work experience preferred, particularly in serving populations such as those with brain injury, intellectual disabilities, autism, or other developmental disorders.

Skills

Essential skills include the ability to assess clients, develop comprehensive service plans, coordinate care, and maintain accurate records. Proficiency in communication, teamwork, and problem-solving is also important.
