Responsibilities
- Provide nursing care including blood pressure checks, blood sugar monitoring, weights, oxygen and nebulizer treatments, catheter care, dressing changes, and basic first aid.
- Administer and document medications in accordance with nursing standards and organizational policies.
- Assist participants with personal cares including toileting, showers, transfers, dressing, and meal setup.
- Participate in the development, implementation, and maintenance of participant care plans.
- Complete nursing assessments during intake and annual updates.
- Maintain accurate documentation, charting, and participant records.
- Communicate with family caregivers, physicians, therapists, and other healthcare professionals regarding participant concerns and care coordination.
- Assist with coordination of therapy services and participant healthcare needs.
- Facilitate participant groups and wellness activities as needed.
- Support the administration of assessments such as mini-mental examinations and geriatric depression scales.
- Cook for and serve meals to participants.
- Coordinate flu vaccinations for participants and Adult Day Services staff.
- Order and maintain medical supplies and nursing inventory.
- Assist with orientation and coordination of nursing students and educational projects.
- Conduct monthly AED checks and assist with VA chart reviews and documentation.
- Educate staff on Universal Precautions and infection control practices.
- Facilitate participant use of wellness equipment such as the NuStep machine.
- Support tours, participant engagement activities, and additional duties as assigned.
Requirements
- Graduate of an accredited Licensed Practical Nursing (LPN) program.
- South Dakota License must be current and in good standing.
- CPR/First Aid certification preferred or ability to obtain within designated timeframe.
- Minimum of 3–5 years of geriatric nursing experience preferred.
- Experience working with older adults, memory care, or adult day services is preferred.

Compensation is commensurate with experience.
Schedule
The role involves frequent interaction with older adults and individuals with varying medical, physical, and cognitive support needs. The environment is team-oriented, fast-paced at times, and requires compassion, professionalism, flexibility, and attention to participant safety and well-being. Schedules may be adjusted to fit program needs.
